What causes "command failed to return results"?

What conditions might cause this error to be thrown: "command failed to return results"? The specific line is at https://github.com/mongodb/node-mongodb-native/blob/1.3.20/lib/mongodb/cursor.js#L681. The error occurs occasionally and I suspect it is related to larger than normal result sets. In what cases would the callback be called without a result *and* without an error?
