Good Accuracy and F Masure but wrong tagging?

I was trying create one NER with NLTK HMM (nltk.HiddenMarkovModelTagger.train). The accuracy of the model is very good so is the f measure(90.00+). But many of my entities are coming wrong or partial match why it is so (though there are many nice ones)?
