Non-use of preserve image urls can be a problem with cache url's ?

If a certain image is deleted from the cache, after she returns to him she getting a new address/file name ? I think of a possible problem, when for some reason the cache address/file name of a particular image will be different in the future, which could cause a problem in cases where there is a link to the same image from an external website. what do you think about that?

I don't fully understand your question.  Which cache are you talking about deleting images from?  How are you deleting them?

Basically, if you turn image-preserve-urls on, then you are not letting PageSpeed extend cache lifetimes by signing URLs.  Instead, the caching behavior should be about the same coming through PageSpeed as it would be if PageSpeed was off.

If you give a more specific example (even hypothetical) with image names and specific mechanisms for deleting them from a cache, I can comment further.
