I was able to reproduce this issue, which only seemed to occur with
the worker mpm, not the prefork one. Anyways, this uncovered a bug
that I fixed and which seems to resolve the issue, and that has been
committed in trunk and the 0.4 branch.

Please confirm if you can try it out...
