I got a error message like this:

Iteration: 5, Log-Lik: -3530.222, Max-Change: 0.00026

Calculating information matrix...
Error in `rownames<-`(`*tmp*`, value = c("v1", "v2", "v3",  : 
  length of 'dimnames' [1] not equal to array extent
In addition: Warning message:
M-step optimizer converged immediately. Solution is either at the ML or
                     starting values are causing issues and should be adjusted.  


What shall I do?

The rownames() error seems odd. Can you provide a reproducible case?

I realized that the error is due to some other code not from the mirt().
