Limit concurrent outbound smtp connections per host, not domain

I'm using Postfix 2.9.6 on Debian Wheezy. I have lowered default_destination_concurrency_limit = 2 from the default of 20, but I'm still seeing 10 or 20 simultaneous outgoing smtp connections towards some hosts. This is a problem, because I get banned with "too many connections".

Those are specifically hosts that handle mail (MX) for many domains. Is Postfix using the domain part of the email address as the definition of "destination" in *_destination_concurrency_limit?

How can I limit the number of simultaneous outgoing connections per actual host, irrespective of the (virtual) domain of the email being delivered, which nowadays means very little?
