No images in nb.html file

At some point recently, plot images stopped appearing in my rendered nb.html files, although they continue to appear in rendered .html files as usual.   I tested the R Notebook template with no changes, and I still have this issue. In addition, at times, the code doesn't appear either, with no show/hide toggle, though it is working now.

I have attached the Rmd, html, and nb.html files (or at least tried to... not sure if they'll go through.)
