administrator/components/com_media/views/images/tmpl/default.php override failing

Is the 'create override' in the Template manager placing the override in the wrong place (administrator/templates/isis/html/com_media/images/default.php) or is the override failing for some other reason ?

Maybe.  Try replacing https://github.com/joomla/joomla-cms/blob/staging/administrator/components/com_media/controller.php#L68-L69 with this:

$view = $this->getView($vName, $vType, '', array('base_path' => JPATH_COMPONENT_ADMINISTRATOR));
