Reazioni da sospensione più frequenti con la Paroxetina che con gli altri
SSRI
Gli inibitori selettivi del reuptake della serotonina ( SSRI ) sono farmaci
antidepressivi ampiamente utilizzati.
Con tutti gli SSRI sono state documentate reazioni da sospensione o sintomi
da interruzione.
Le segnalazioni di reazioni avverse provenienti dal Canada, Australia,
Francia, Gran Bretagna ed Usa, mostrano che l'incidenza di reazioni da
sospensione è più elevata nei pazienti che fanno uso della Paroxetina ( in
Italia: Sereupin , Seroxat , Daparox ) che con altri antidepressivi SSRI.
Sebbene il meccanismo della sindrome da sospensione degli SSRI non sia
completamente conosciuto, l'esordio, la frequenza e l'intensità dei sintomi
sembra variare in base alle proprietà farmacologiche, farmacocinetiche del
farmaco, come l'emivita ( il principale fattore di rischio ), la presenza di
metaboliti attivi e gli effetti anticolinergici.
Rispetto agli altri SSRI, la Paroxetina ha una breve emivita, nessun
metabolita attivo, un più pronunciato effetto anticolinergico ed una
maggiore potenza nel bloccare il reuptake ( riassorbimento ) di serotonina.
I sintomi da sospensione possono presentarsi dopo interruzione del
trattamento, con la riduzione del dosaggio, con il passaggio ad un altro
antidepressivo, o per semplice dimenticanza nell'assunzione del farmaco.
Con la Paroxetina i sintomi da sospensione si evidenziano rapidamente.
I sintomi osservati dopo sospensione di un antidepressivo SSRI possono
essere fisici o psicologici e rientrano nelle seguenti categorie:
alterazioni dell'equilibrio ( capogiri, vertigini, atassia ), disturbi
gastrointestinali ( nausea, vomito ), sintomi simil-influenzali ( senso di
affaticamento, letargia, mialgia ), disturbi del sensorio ( parestesia ),
disturbi del sonno ( insonnia, sogni vividi ) e disturbi psichiatrici (
ansia, agitazione, confusione ).
I sintomi da sospensione possono facilmente essere interpretati come il
ripresentarsi della depressione, o inefficacia del farmaco antidepressivo.
La maggior parte delle reazioni da sospensione sono lievi e transitorie; si
presentano entro 1-3 giorni e durano 7-14 giorni.
Solo occasionalmente i sintomi perdurano per diverse settimane. Tuttavia
alcune reazioni possono essere gravi e richiedere trattamento in acuto.
Le Autorità Sanitarie Canadesi ( Health Canada ) hanno ricevuto fino al 31
ottobre 2002, 102 segnalazioni di sospette reazioni da sospensione:
Paroxetina ( 79 ), Sertralina ( 10 ) , Fluoxetina ( 6 ), Citalopram ( 5 ),
Fluvoxamina ( 2 ).
Il 49% di queste reazioni è stato classificato come grave.
Per prevenire o ridurre la sintomatologia associata all'interruzione dei
farmaci antidepressivi SSRI viene raccomandato di diminuire gradualmente la
dose.
Alcuni Autori suggeriscono di assumere la Fluoxetina se i sintomi diventano
gravi ed il paziente non è in grado di sospendere l'SSRI nonostante la
riduzione del dosaggio.
Per la Paroxetina, alcuni consigliano di ridurre la dose di 5 mg/die ad
intervalli settimanali.
L'Health Canada segnala che l'impiego di farmaci antidepressivi a breve
emivita, come la Paroxetina, rappresenta un importante fattore di rischio
per le reazioni da sospensione.
I pazienti dovrebbero essere informati dei rischi delle reazioni da
sospensione prima di intraprendere il trattamento. ( Xagena2003 )

Paroxetina nell'occhio del ciclone
Fino a ieri tanto osannati ora nell'occhio del ciclone. Sono
i farmaci serotoninergici (SSRI), fortemente sotto accusa
non solo in America ma anche nel nostro paese.
Si tratta, in particolare, del Plaxil americano e del
Seroxat e Sereupin italiani, finiti sotto accusa per gli
effetti collaterali e la possibilita' di determinare
dipendenza.
La prima denuncia italiana contro il Seroxat, un
antidepressivo della Glaxo Smith Kline, fa seguito alle 35
in California sporte da pazienti che hanno gia' fatto causa
alla stessa ditta con l'accusa che tale sostanza crea
dipendenze pericolose.
Sull'onda di questa denuncia, due italiani hanno fatto
pervenire al procuratore Guariniello due esposti chiedendo
che si faccia luce al riguardo. Entrambi i pazienti italiani
hanno dichiarato di aver subito, dal momento dell'assunzione
di pastiglie di Seroxat, un aggravio delle condizioni di
salute sicche', anziche' essere liberati dal male oscuro, vi
sono ripiombati.
E' pur vero, pero', che tali sostanze preposte alla cura
delle forme depressive, del tono dell'umore, del disturbo da
attacchi da panico, fino al disturbo ossessivo compulsivo e
alla fobia sociale, vengono eliminate dall'organismo in
tempi ristretti, dalle 24 alle 36 ore e, quando si sospende
la terapia, e' naturale l'innescarsi di sintomi da
astinenza.
Di conseguenza, avvertono gli addetti ai lavori, la
sospensione deve necessariamente essere graduale, onde
evitare gli stessi sintomi denunciati dai pazienti, ossia
sbandamenti, mal di testa e vertigini, stati confusionali,
ansia, insonnia e nausea.
Riducendo gradualmente il farmaco, invece, si evitano simili
disturbi che, comunque, secondo i piu' accreditati
neuropsichiatri, sono transitori. Adoperata correttamente,
dunque, la paroxetina non dovrebbe determinare alcuna
dipendenza ma e' proprio questo che vuole accertare ora il
procuratore Guariniello, soprattutto per evitare un altro
caso Lipobay.
Per tale motivo e' stato richiesto al Ministero della Salute
di trasmettere tutte le note tecniche fornite dalla casa
produttrice anche per conoscere se, in passato, vi siano
state segnalazioni al riguardo.

Hi, first-time poster...
I've taken 40 mgs/Paxil for the last several months and have been doing
very well, a lot more like my old self mentally. I've approached tasks with
a lot of enthusiasm and I've had some decent bursts of creativity lately,
too, all in things that seemed totally meaningless to what I was without
(namely, the lady love I lost).
Anyway, I was cruising along until I ran out, w/ no refills, of Paxil about
four days ago. My doc is in the process of moving into a new bldg., and
getting in touch with them has been a real bugger of a thing to say the
least. The second day w/out Paxil I experienced a strange sensation while
standing, a kind of seizure wherein I felt an almost electric force surging
through my head, but it was only for a fleeting moment. Then, the
next day around 4:30, a profound sense of being really hot and dizzy
overcame
me, and I rested for awhile but couldn't shake the feeling. Now, today,
I can't even describe what kind of things I'm feeling--VERY dizzy at times,
with a weird thumping/pounding feeling in my ears and behind my eyes when
I look around or forcefully close my eyes.
I managed to get my dosage back in me again several hours ago, but if it's
some
kind of extreme withdrawal, then the drug hasn't helped yet. Whatever this
is,
it's very scary, and has me wondering if something very serious is wrong
with
me--some damage to my inner ear, I don't know what else. Is there any record
of sudden Paxil withdrawal causing some rather wild physical side-effects
as well as emotional? I have been much more prone to bursting into tears
at very small things in the past 36 hours...even my "sex drive" returned
after
remaining in rather dubious condition for the majority of the time on Paxil.
But I would gladly trade that to get rid of this God-awful feeling in my
head.
Please, any advisement/help is appreciated; I realize this is a rather
rambling
and incoherent account, but the stuff with my ears is starting again and
I feel as if I should wrap it up...

That sounds normal for Paxil withdrawl. At least it sounds
like the way mine was. It will keep getting better as you
continue the medication. Just keep taking it, the side
effects will go away. Don't ever ever ever quit Paxil cold
turkey. Ever.

I have heard of dizzyness associated with going off a drug like
Paxil "cold turkey" did you tell your doctor about this? It could be a
combo of withdrawl and nervousness that you have at being without the
medication that you aren't even aware of. I went through something
similar when I went off of Prozac to switch to a new med. I don't think
what you wrote was scattered or rambling at all. You sound pretty
together to me, just a little shakey.

My doctor tells me that there will be no symptions of withdrawal when I
stop taking 20 mg of paxil a day.
This does not seem possible. What does paxil do? It seems that any
habitual intake of chemicals would cause some form of withdrawal,
especially where emotional well-being is concerned.
Is paxil really just a placebo to provide an emotional crutch during times
of intense difficulty? If not, how can there be no withdrawal symptoms?

While some antidepressants such as Prozac often have no
withdrawal symptoms following abrupt discontinuation,
Paxil and Effexor are two antidepressants that frequently
have a severe and lengthy withdrawal syndrome. Such a
withdrawal syndrome is not seen with Prozac because it
reamins in the body for about 5-weeks after being stopped.
Paxil and Effexor are totally excreted in about 5-days.
Among the symptoms that are seen in a Paxil withdrawal
are:
Nausea
Vomiting
Weakness
Tremors
Pins & needles sensations
Visual distortions
Diarrhea.
I tell my patients when discontinuing Paxil to lower the
dose by steps of 5 mg or 20% of the total daily dose,
whichever is less. Eash step should be for 5-7 days to
minimize the probability of withdrawal symptoms.

The only side-effects I experienced from withdrawal of 20mg Paxil was
a vertigo sensation when I turned my head too fast.

Someone needs to document this effect. I had the EXACT same experience
with disorientation when I turned my eyes (not my whole head). It was
pretty scarey and it lasted a couple of weeks, maybe three. I was SO
relieved when it finally quit.

This is weird! I've experienced the same "whoosh" with eye movement when
I've come off of Nardil and Parnate. Obviously, it's not confined to
SSRI's. I've felt it's more like a "zhoomp-zhoomp" sound. I told someone
that it felt as if I'd turned my head and it took a split second for my
brain to turn with it--sloshing around! ;)

My experience with Paxil withdrawal was with a sensation. This sensation
was very infrequent during the last days of 1/2 dosage and became more
frequent within a day of zero dose. From day 2 to day 5 after stopping
Paxil completely, I would have the sensation as frequently as 3 times a
minute. I am on day 10 now and have the sensation only around once every
few hours. Turning my head did frequently trigger the sensation, as did
standing up too quickly after sitting for a while. Also, standing in
one place would cause more frequent sensations. Most of the time, there
was no trigger.
The sensation is like nothing else I have ever had. There is dizzyness,
a quick flushed feeling, disorientation, almost a blackout. A few times
I could hear blood pounding in my ears. Also, a few times I needed to
hold on to something for balance.
I would tend to have the sensation less in the morning and then more
frequently as the day went on.
Two days after I did, a friend of mine also went off Paxil and
experienced all the same symptoms. He said an acquaintance of his had
similar symptoms, which he described as pressing CTRL-ALT-DELETE on his
brain once a minute.
I am curious as to whether there is any description of these symptoms in
the medical literature. This cannot be too uncommon a withdrawal symptom.
As my friend said, "I know three people including myself who went off
Paxil and all three had the same symptoms."
I am also curious about whether these withdrawal symptoms could be
dangerous for patients with other preexisting conditions, such as cardiac
conditions, circulatory problems, tendency to have seizures, etc.

Se la cura ha funzionato e non ne ha più bisogno, l'unica cosa è
sopportare la sindrome di astinenza.
Credo che lo psichiatra sia in difficoltà, una sindrome così forte
credo sia poco comune.
Potresti chiedere allo psi di informarsi meglio, soprattutto su quanto
dura.

E che c'è da avere paura? La sindrome di astinenza è una malatia come
un altra.
In più hai la certezza di guarigione. In caso di astinenza da oppiacei
potrei dirti con certezza tempi e sintomi, di questo caso invece non
sò nulla.
Io ho utilizzato questo farmaco per diverso tempo (un anno e mezzo/due
anni).
Ti posso dire solo di stare tranquillo, tutto tornerà alla normalità.
L'errore che molto spesso si fa in questi casi è farsi vincere
dall'ansia e evidenziare in modo marcato gli effetti della sospensione
che a volte, come è successo a me,
ci sono ma transitori e se ne andranno in poco tempo.
Tieni duro e abbi fiducia nel tuo medico... in alternativa prova a
contattarne un'altro, per farti stare tranquillo, no?
