gRFC A9: Server-side connection management

Discussion for https://github.com/grpc/proposal/pull/23 to have configuration for a variety of server-side connections management. This is for cleaning up connections and forcing new ones for L4 load balancers.
Like A8 (but to a lesser degree), this design also pre-dated the gRFC process.
