ultradocs not producing

Hi Daniel,

 

I’m having some issues with using ultradocs again…you’ve helped in the past and I’m wondering if you can do so again!

 

An ultra-doc that my colleage (Tom – CC’d)and I created, which worked fine then, is no longer working.  It goes as far as creating a document, but most of the info to be inserted is missing and/or in the wrong spot.  No emails are being generated either.

 

Here is a screen shot of what I’m referring to.  Please let me know if you are able to help me solve the mystery.
