How can i create a mail merge with a customized google form by an spreadsheet?

I have a list of people but for each person i gotta ask them about other people filling a form. I got the link between the person that im going to ask and the people for which i want to ask. I want to do it using a google form created by a spreadsheet, and for each form send it embeded with the email. How can i do it?
