Using the Grid in Google Form to format a table to generate a quote

I'm using a Grid to select the Product type (on a column) VS the Attribute (on the row) to retrive via a Lookup table the corresponding price of the combination, but I'm struggling to find an easy way to do this.

Could you please advise how I should do this?
