Ultradox not connecting to google form

Hello, I'm having an issue with Ultradox's google form integration.

I'm trying to load data from the google form. but it is stating I don't have the form trigger add-on installed, which I must have as I started the flow from the form trigger add-on's sidebar. is there something I'm missing?
