Map-enabled web form for collecting location inputs

Hi Friends,

With some help from Sajjad, I've created a proof-of-concept for a map-enabled web form for collecting location-based inputs, that works directly with Mapbox SDK API. It does not need any server setup and can run from static HTML page too. (needs some protection for hiding the accesskey though)

I've used a cool tool called StatiCrypt for encrypting the demo page.
