STC Hants - forms for 2018 courses

Good morning Richard and Tony,
Tomorrow is the day that I told the pupils I expected to have the
application forms ready to send out.

I have had a look at past forms, the attached text file is a summary of
what I saw, it is just to jog my memory. I will add to it what forms I
sent out and if I vary it depending on the ages of the pupils.

One thing caught my eye from the distant past, I found this in my report
on the 2004/5 seasons - "Rockbourne were not keen to run another course,
they were not impressed by our helpers last time, who appeared to be
there to fish for themselves". We have discussed this sort of thing
quite recently, but that's a digression.

I am still a bit unclear exactly what forms Colin sent out and who
should receive which form. The main form that everyone gets is currently
called "BoookingForm2018.pdf" you have already seen a draft version and
the instructions in "HowTo.pdf". The booking form is based on Colin's
form "Application Form Family.doc" which I have changed as we discussed.
I am still fine tuning it and will send you a copy when it is ready.

Colin has a form "Application Form Juniors.doc" which I take to be for
junior courses and therefore I presume not used. Not using it gives rise
to issue (1) below.

There are many versions of the parental consent form, the most recent is
the angling trust one which I plan to use. I will send it to those
people who which to bring their children to us and leave them in our
care for the day as pupils - see issue (2).

Colin had "Notes for parents (word).doc" which I am not sure if he used
as quite a bit is repeated in "Directions and Instructions" for each
course. I have prepared directions for getting to Holbury and changed
the Longstock directions. I am planning to have one "instruction"
document and no instructions in the individual fishery directions. I
will send you a copy of these instructions when they are ready.

You two are the experts in child protection and risk assessment, if you
see anything I am doing wrong or could do better please let me know.

I propose to send the first batch out either tomorrow night or Saturday
morning. If I leave something out we can always go back to the pupils.
It is probably best to keep up the momentum and not try to get
everything perfect the first time around. Please take a look at these
issues and be ready to react if you think I am about to do something stupid.

Possible issues:
(1) Should we ask ahead of time for permission to use photos or ask for
it when we wish to use them? What did we do for the Total Flyfisher article?
(2) Am I right to assume that it is only necessary to fill in the
parental consent form when junior pupils are left in our care without a
responsible adult?
(3) We will accept under age pupils at our discretion after discussion
and they must be accompanied by an adult. Some of our literature says
"responsible adult" others say "participating adult". Colin told me
"participating" meant being a pupil on the course and he liked to
enforce it. I know we have had watching grandparents in the past and I
propose to use "responsible" throughout. Any problems with that?
(4) Tony suggests we should have a question about "health issues we
should know about" for adults and juniors such as being in a wheelchair
or having an oxygen cylinder strapped to your back. I plan to have a
check box that says "tick me if anyone in you party has something like
this so we can discuss it". Well something like that anyway.
