Best way to do microtonality – use a softsynth that takes Scala files. Then you can send unadorned MIDI note numbers to the softsynth, and the softsynth handles the tuning for you.
is the source for Scala. Scala also comes with an archive of over 4000 microtonal tuning files.

As well, on the iPad, the sampler program Thumbjam handles Scala files very well. Also on the iPad, there are the apps ScaleGen and Wilsonic that will make microtonal scale patterns for you and you can export the output of these programs as Scala files as well.

I seem to recall that the old Yamaha TX 81Z had micro tuning capabilities, but I don’t recall how that was achieved. There must be a manual for the module on the web.
Warren Burt would surely know all about alternate tunings and MIDI.
