Project

Telerik report generation in support of the rewrite and platform refresh of the Food Safety Inspection Licensing System (FILS) will be the primary focus. The legacy food safety applications are coded in MS Access 2007 connected to SQL Server 2008 R2 databases via an OLE connection (.adp).  The corresponding SQL databases contain complex stored procedures, views, triggers and functions which must be evaluated, modified and created.  Programmer should have proficiency with SQL and interfaces (SQL Server Manager, SQL Studio Management). Food Safety employees manage regulatory, licensing and inspection functions via two complex compiled MS Access project (.ade).

The programmer must fit in teamwork environment as well as follow direction.  Programmer may be responsible for all phases of the Software Development Life Cycle (SDLC); gather project business requirements, analysis of hardware/software requirements, design forms and reports, database modifications, code review, unit testing, user support and training, and maintenance.  Development will be performed in a test environment and promoted to production environment upon approval by senior developer or supervisor. Programmer must use established formats, review and understand existing code, controls, SQL stored procedures and views.   The programmer must adhere to agency coding standards, procedures, and complete documentation such as impact analysis and change control. Use of third party tools is prohibited without prior approval by the project programmer or supervisor.  Programmer will participate in weekly developer working group meetings to discuss progress, obstacles, timelines, and goals.

Position Summary:  Surgery coder I position is responsible for applying appropriate medical billing codes to select, simple operative reports and/or chart notes, applying diagnosis codes to operative notes and chart notes, and auditing pre-coded fee tickets in accordance with medical billing industry laws and regulations.  Surgery coder I is also responsible for appropriately responding to coding questions asked by clients or MedFocus staff and maintaining surgery logs and other documentation in good professional order. 

 

Must have one year of coding ICD-9 and CPT coding experience.  National coding certification or registration is required.  Staff in this position will be screened for sanction on the published General Services Administration’s List of Parties Excluded from Federal Programs and the HHS/OIG Cumulative Sanction Report.

 

Key Functions and Accountabilities:

  1. Retrieve operative reports / office chart notes via an electronic method or paper format from client, if necessary.
  2. Assess physician documentation and assign appropriate and lawful procedural (CPT), diagnosis (ICD-9), medicine/supply (HCPCS) codes and modifiers in accordance with all applicable medical billing industry laws and regulations, including specific CMS guidelines and Correct Coding Initiative (CCI) edits.
  3. Assign ICD-9 codes accurately using the highest level of specificity permitted.
  4. Maintain adequate production based on number of clients assigned, client specialty, client level of difficulty, and other tasks assigned.  
  5. Correspond with assigned client regarding individual documentation or coding questions in a timely and professional manner.
  6. Correctly answer coding questions to clients or internal staff in a timely manner.
  7. Report to Coordinator all concerns related to client documentation, client-provided codes, client requests or directions given to MedFocus to Coordinator that are outside of the scope of MEDFOCUS services, or related to potential inappropriate or incorrect billing.
  8. Fully cooperate with any audit or request for information.

Job Title:                    Surgery Coder II

Department:              Coding Department

Reports to:                 Coordinator of Coding Department

Position Summary:  Surgery coder II is responsible for applying appropriate medical billing codes to operative reports and/or chart notes for general surgery and three specialties in accordance with medical billing industry laws and regulations.  Surgery coder is also responsible for appropriately responding to coding questions asked by clients or MedFocus staff, creating written coding articles, and maintaining surgery logs and other documentation in good professional order. 

 

Must have between one (1) and four (4) years of coding ICD-9 and CPT coding experience.  National coding certification or registration is required.  Staff in this position will be screened for sanction on the published General Services Administration’s List of Parties Excluded from Federal Programs and the HHS/OIG Cumulative Sanction Report.

 

Key Functions and Accountabilities:

  1. Retrieve operative reports / office chart notes via an electronic method or paper format from client, if necessary.
  2. Assess physician documentation and assign appropriate and lawful procedural (CPT), diagnosis (ICD-9), medicine/supply (HCPCS) codes and modifiers in accordance with all applicable medical billing industry laws and regulations, including specific CMS guidelines and Correct Coding Initiative (CCI) edits.
  3. Assign ICD-9 codes accurately using the highest level of specificity permitted.
  4. Maintain adequate production based on number of clients assigned, client specialty, client level of difficulty, and other tasks assigned. 
  5. Correctly enter surgeries billed into MedFocus Surgery Log. Upon request from client, provide a printed copy of the MedFocus Surgery Log.
  6. Correspond with assigned client regarding individual documentation or coding questions in a timely and professional manner.
  7. Create six written coding articles per year.  These articles will be e-mailed to Coding Coordinator in a written professional format and used either for the MedFocus Newsletter or inter-office education.
  8. Correctly answer coding questions to clients or internal staff in a timely manner.
  9. Report to Coordinator all concerns related to client documentation, client-provided codes, client requests or directions given to MedFocus to Coordinator that are outside of the scope of MedFocus services, or related to potential inappropriate or incorrect billing.
  10. Fully cooperate with any audit or request for information.
  11. Provide all services with the highest standards of ethics and honesty. 
  12. Comply with the standards of all MedFocus policies and procedures, including the Compliance Plan, HIPAA Policy and all associated documents. 
  13. Other related duties as they are assigned by your Coordinator or Owners of MedFocus from time to time. 

 

Production Requirements:

  1. Documentation coded within 72 hours of receipt.
  2. Surgeries logged into MedFocus Surgery Log with 98% accuracy.
  3. Coding accuracy rate at 90% or greater based on CMS guidelines and CCI edits.  Random audits will be conducted to ensure accuracy rate.

 

Security, Ethics and Compliance:

  1. Must comply with the regulations of the Health Information Protection and Accountability Act (HIPAA), and all other state and federal laws related to patient confidentiality.  Position may also have access to proprietary MedFocus information, and as such, requires adherence to MedFocus Confidential and Proprietary Information Agreement.
  2. Must act prudently and in a timely manner to comply with the MedFocus Compliance Plan and associated documents.   
  3. Maintain an open line of communication with Coordinator or other MedFocus leadership to report activities in conflict with the MEDFOCUS Compliance Plan, or other criminal or unethical activity at MedFocus or by clients. 

 

Challenges Unique to This Position:

  1. Assigned codes, and therefore work produced, is highly regulated by industry laws, guidelines, and regulations with significant civil, legal and monetary penalties for intentionally, knowingly, or willfully violating such laws.   
  2. High visibility, close relationship and accountability with MedFocus clients.
  3. Requires firm adherence to correct and timely processing.

 

Skills and Experience Required:

  1. National Certification or Registration is required (CPC or ARHCP);
  2. High School Diploma required;
  3. May not be excluded or ineligible to participate in Federal healthcare programs, suspended or debarred from Federal government contracts, or have been convicted of a criminal offense related to the provision of healthcare items or services and have not been reinstated in a Federal healthcare program after a period of exclusion, suspension, debarment or ineligibility;
  4. Must have between one (1) and four (4) years of ICD-9 and CPT coding experience.

  10. Conduct professional quality coding education meetings with clients or internal staff at the request of Coordinator.
  11. Calculate CPT fees using the conversion factor and RBRVS relative value units;
  12. Update billing information system with CPT and ICD-9 codes and fees in the absence of Coordinator.
  13. Report to Coordinator all concerns related to client documentation, client-provided codes, client requests or directions given to MedFocus to Coordinator that are outside of the scope of MedFocus services, or related to potential inappropriate or incorrect billing.
  14. Fully cooperate with any audit or request for information.
  15. Provide all services with the highest standards of ethics and honesty.
