Consent Form & Pre-Program Questionnaire

I have attached:1.) the consent form from Liz D'Imperio for Open Airways for Schools in New York City, and 2.) the pre-program questionnaire. Both of these need to be given to the children/parents or guardians before we start.

 

Remember, we had discussed merging the spirometry testing and the home environmental testing with the OAS consent form.
