marriage and baptism record form

I've never studied Portuguese but have Spanish and French and with that I can make out some of the records. But it would be easier if I knew what their standard format was for marriages and baptisms. Sometimes there are so many abbreviations and loops in the letters I can tell who is who and what is what. Also perhaps a handy list of common abbreviations for names and acts. 

So like date/month/year, place?, church?, priest?, name of baptized? etc. Also on baptisms whose name is in the margin, the baptized or the father? Thanks.

For whatever reason, I can't get the Azores GenWeb to load. I don't know if it's on my end or not. I will try again later tonight (I'm in Pacific time) to get it to you.

The list of keywords is here. You are in the 2nd time period. That's what you want to focus on.

At the end is examples from all 3 times periods. You want the 2nd time period.
