Play predefined audio file during a live voice call such that the other end can hear it

I want to play a  predefined audio PCM file when an incoming or outgoing call happens such that the other end can hear it. I know there are no API which support this but is there any other way to achieve this such as modifying the android source code or doing something at the low level API or anything else.
