Append Value (from Creative Input box) in Destination URL

Hi Adwords Folks,

Good day! Would you anyone share their experience whether my desire functionality is feasible.

My client is looking for a functionality that has a dynamic destination url, where there is a input text box in the creative. Whenever the user click the CTA it automatically append the value inside the input box to the original URL something like (e.g. http://www.sampleURL/?id=12345, assuming the 12345 is the value of the input box. Would this possible?

Attached files I my working sample, I make it work locally but not in Adwords UI; files is made in GWD.

Any insights is much appreciated.
