[MT-L] Richmond Agitation Sedation Scale (RASS)

We are CS 5.67 PP3

 

I want to remodel how our RASS Assessment is set up.  Currently, it looks like this due to character limitations on elaborating the different agitation/sedation levels.  Each is assigned a score from (-5) to (+4).  I want to include a rule that will only allow the user to select ONE answer for the entire assessment; but I haven’t been able to locate one.  Do any of you have something that would help?  Or do you have a better RASS model you could provide?  I’m open to any suggestions.
