Vagrant can't use the requested machine because it is locked

My apologies if this issue has been addressed.  I didn't find anything using the search function.

I am unable to run 'vagrant up'.  I always get the error shown in the subject.  I have a debug log at https://gist.github.com/anonymous/bc4a85e67bb925c7fc20

I don't see any hints about the nature of the failure in the log. 

If anyone has suggestions, I would be most grateful.

I have tried:
- run vagrant destroy services
- delete .vagrant
- remove & add the relevant box
- clear all existing VMs from VirtualBox.

environment:
Windows 7
Vagrant 1.8.1
VirtualBox 5.0.12.104815

Check the VirtualBox gui if there is any VM in the GUI left, and in the media manager is there is any orphan disk.

Be sure to be running Vagrant as your normal user, and not as Administrator.

There are no other vagrant or VB processes running.  I cleared out all the VMs from VirtualBox & there are no disks shown in the Media Manager.
