Following advice from the Commission on Human Medicines, the Medicines and Healthcare products Regulatory Agency (MHRA) has reclassified Cialis Together so that it can now be made available for purchase over the counter, without a prescription.

Dear Colleagues

I have a 64 year old lady wanting to rekindle a sexual relationship with her husband (68)

She has seen a specialist private doctor who has prescribed

Dosage :- 2 pumps of Estradiol Oestrogen gel daily

1 pump of Tostran Gel 4 times a week

1 Ustrogestan 100mg oral tablet every night

1 Vagirux (Estradiol) 10 micrograms 3 times per week

Side Effects:- Enlarged, sore breasts, enlarged sore nipples with some slight discharge

                      Sensitive skin

                      Whitish discharge from Vagina

                      “Brain Fog”

                      Similar feelings to PMT

Her last blood tests(11/1/23) showed a Testosterone level of 0.9SHBG 77 and free androgen index of 1.2

What changes can she make to alleviate her symptoms which are affecting her compliance with treatment?

Her previous history is elective surgical menopause at age 50 bilateral oophorectomy. No T treatment.  History of endometriosis, 2 rounds of IVF but failed to conceive and now has 2 adopted daughters.  One of which causes considerable stress as she has severe mental health issues.

Complains of deep pelvic pain and her last smear was incredibly painful.  She has used dilators under my guidance with some degree of success and she is keen to move on in her sensate focus programme.

Hi Mary, a lot to consider and change with this regime for a basic email but if that’s her T result when on Tostran then she either isn’t applying it correctly or she isn’t absorbing it as it should be much higher if she is on Tostran.
If she is being prescribed through a private menopause doctor then they should adjust the products and doses accordingly.
There is a lot that could be altered here but if she’s already under a specialist then you might be better liaising with them.
Best wishes, Trudy
