Request for advice re:temporal lobe epilepsy/limbic encephalitis

Hello everyone, I seem to have disappeared from the SSSSR communications but I am back, thank you Kevan. I hope 2025 has started well for you all.

The reason I am asking is that I have been approached by a 41 year old man diagnosed 20 years ago with temporal lobe epilepsy/limbic encephalitis and he is looking for medical advice on how to minimise the impact of this and his treatment on sexual function - specifically desire for sex. He feels he is not having much luck with his Neurologists.  Do you know of anyone practicing who I could approach or could you please share any other information you feel could be relevant and I can pass it on.

Hi Julie, has he had his testosterone checked as we know epilepsy impacts on hormone levels. Even if they’ve been done I’d want to see the actual results as many GP’s follow lab results of anything above 8 being normal.  As you know with symptoms we would explore treatment if below 12 following BSSM guidelines. 
That would be my first point of call before working with PST. Happy to discuss more if helpful. Thanks, Trudy

Hi Trudy,

Thanks for this response, and to Kevan for posting my initial query.  The enquiry came from someone looking for a medical approach, so at this stage there is no intention to work with him in PST.  Would this be an issue your centre would be able to address?  His query is specifically related to desire and the impact on this of his condition and treatments.  He has researched kisspeptin and I think he is looking for a medic who can talk confidently to him about his sexual response and whether there is any additional treatment or information for him.

Hi Julie

I was working here at RHH with a younger man with epilepsy and his loss of desire was highlighted to me to be as a result of the medication he was taking - Tegretol (carbamazepine). His consultant was willing to look at alternative medication options as he was in an arranged marriage and the issue was causing distress. The process of changing medication was going to be quite long-term and he was advised that the loss of desire might persist even if he changed medications

Hi Peter,

Thanks for this.  Yes the experience sounds similar to this person, his medication has already been changed once but with no improvement.  He is finding it difficult for anyone to talk openly about sex with him and what his options are so it might be helpful for him to consider the Leger clinic.  I wasn’t sure that they took on this kind of work so helpful to hear otherwise.
