SMART on FHIR Patient and User scope difference

I might be asking a very basic question, as per my understanding,
Patient Scope e.g. "Patient/observation.read" allows a single patient to read the observation data.
User Scope e.g. "User/observation.read" allows users to read the data of multiple patients at once in the app.
  • Can a practitioner type of user(access to multiple patients) be granted patient scope?
Please add any pointers to list out the difference between the patient and user-level scope

I think that should be fine for a practitioner if its is routing through an app which takes care of consent workflow. 

User is primarily thought to be linked to Patient, but the other way to think about it to see user having access to patient. 

Thus user maybe patient themselves, or family member or practitioner. 

The only catch is you need to be sure on how the access has been taken and granted.
