Fields with a mixture of radio buttons/text boxes and text?

Hi all,

I was wondering if there was a way to make a field where I can input text OR be able to check something off. So, for example, if I want to record a participants hemoglobin level, I would enter the number in the field. However, if they did not have labs drawn for that specific instance, I could check off "labs not drawn" or something. The only way I can think of doing this is just making it a free text field without any validations and including a field note to say "Labs not drawn" or something if that is the case, but I would prefer to do it in a way that is cleaner and more concise.

How about using field embedding to give you something like this?

 

You could have branching on the date field to hide it when the "Unknown" box gets ticked.

Luke’s suggestion of embedding fields, is what I would do as well; however, one thing I’ve also done for some projects is to use missing codes like “NA” (or you can even create your own) and input that instead.
