How about using field embedding to give you something like this?

You could have branching on the date field to hide it when the "Unknown" box gets ticked.

Hi all,
I was wondering if there was a way to make a field where I can input text OR be able to check something off. So, for example, if I want to record a participants hemoglobin level, I would enter the number in the field. However, if they did not have labs drawn for that specific instance, I could check off "labs not drawn" or something. The only way I can think of doing this is just making it a free text field without any validations and including a field note to say "Labs not drawn" or something if that is the case, but I would prefer to do it in a way that is cleaner and more concise.
