v4.0.3 install hangs at "Creating ext4 at .../qubes_dom0-root" then fails

Hi,

I wasn't sure how to send the bug report from my installation attempt (to a ftp or whatever). If someone tells me how to do that I will. The details of the error message are too numerous to copy by hand and then type in here! If nothing else maybe I could capture the text using OCR on my phone.

I've been trying to install v4.0.3 from one USB 3 stick (SanDisk) to another USB stick (Alcor) on my Lenovo Ideapad 310 laptop (meets all requirements except SSD). The install appears to progress normally until the status message above the progress bar reads "Creating ext4 at .../qubes_dom0-root," at which point the install hangs for a long time. Finally there is a generic error message. The details of that message lead me to believe that at least part of the problem is a failure to format the destination USB drive. Interestingly, at first when I booted from the installation USB stick a page of "boot text" would appear, beginning with "Xen," etc. before the install GUI became visible, but this no longer happens. I've been using the Legacy BIOS boot option, not UEFI.

Any assistance will be greatly appreciated!
