USB Forwarding in Qubes R3.1

I am unable to assign USB devices to AppVMs in Qubes R3.1. When I assign a
USB device to an AppVM via the Devices tab in the Qubes VM Manager, it
does not show up in lsusb in the AppVM. I have tried this on the fedora-23
and fedora-23-minimal templates, and disabled memory balancing in both.

I am also unable to assign USB devices to dom0, as it does not have a VM
Settings option in its drop down menu in the Qubes VM Manager.

How do I assign USB devices to AppVMs and to dom0?

Your usage of the term "USB device" is ambiguous between:

1. USB controller

and

2. USB mass storage device (block device), USB mouse, USB keyboard, etc.

The "Devices" menu is for assigning the former to a VM, not the
latter. I'll use "USB device" to refer to the latter.

If possible, you should assign your USB controller(s) to a USB qube
(aka USBVM). Then, when you physically attach a USB device to your
physical machine, it should appear in the USB qube. You can then use
Qubes Manager (or the qvm-block command) to attach it to any VM. (At
least, this is how it works in the case of USB mass storage devices.
USB mice and keyboards are a bit different, as explained on the page
linked above.)
