USB A to 2 input issue

I have an issue with the USB A to 2 input adapter, I bought it from the QS website. Anyway when I plug a switch into it by itself or with a breakout adapter, it's behaving like the switch is constantly being pressed, it will also behave like this with nothing plugged in. It was working fine when I 1st got it, but just started doing this the other day.

I have an FPS QS, it's running the latest shipping firmware. I'm trying to use the adapter because I am already using all the other inputs.

Send in a debugging report with QMP:

1. Plug the Quadstick into the PC
2. Start QMP
3. Wait 20 seconds
4. Click in the Transcript pane at the bottom of the window.
5. Press F1 key
6. Answer prompt to send in the debugging report
7. EXIT QMP.  Ignore any error message.
   
8. Send me an email so I know to go look for the report.
