QAP doesn't escape ampersands in folder and file names

In Folder menus and menus  like "Recent Folders" QAP fails to escape folder names, so that folder "A & B" appears in the menu as "A B" and folder "A && B" appears as folder "A & B"

So if a folder contains the following folders:

"A B"

"A & B"

"A && B"

A dynamic menu based on that folder contains the following:

"A B"

"A  B"

"A & B"

I understand this behavior for menus we explicitly create...but I don't think this makes sense for menus based on content we may have no control over.

I can reproduce it. Good catch. I hope we would have found this one before :-(
