dear diane and others,

i have read on several of the chats on the liver and pkd discussions that some of you suffer from recurrent UTI. one very effective way to not have it recur, is to look after the health of your gut and urinary tract. unfortunately the usual way to treat UTI is for your gynecologist to give you antibiotics. although this helps to kill the bacteria but it compounds the problem by killing off all the good microbes that in the long run help to keep UTI at bay.

Antibiotics alone is not the answer. many have had a much better result with using probiotics. i am not saying that one does not take the antibiotics for any infection but one must follow it up with probiotics and not for just a while but as part of your prevention regime you should take it continually so that you have the good microbes preventing the bad bacteria from taking hold.

google 'fem dophilus' and you will find a supplement which contains 2 important strains of probiotics lactobacillus rhamnosus GR-1 and lactobacillus reuteri RC-14 that have over 25 years of research and clinical studies done on these 2 strains that have proven to ensure urinary  tract health, all very authoritative. aside from taking probiotics orally, some have used probiotics as vaginal suppositories and they get quick relief.

try it and you will not get recurrent UTI

Dear Cath
I appreciate all the research you have done.  It maybe very, very, helpful for someone younger.  As someone who is 73 y/o I do suffer from frequent UTI.  So does my urologist!

I have done all the things recommended for UTI, but to no avail. Once they started I would get a UTI every 3-4 months.

Over the years because I have severe PLD I have kept my exposure to even food sources of hormones to a minimum.

My GYN urologist said I needed hormones.

I asked my nephrologist. He said because of my liver, my PLD, he thought no hormones, instead I was to take 125 mg keflex each night. This is a very small dose. I have found it does not affect my gut bacteria.  This too is the experience of my nephrologist.

I have continued to be seen by the Women’s Health Clinic at the Mayo.

Finally I was seen by a young female doctor who recommended 4 micrograms of estradiol called Imvexxy. It is used twice a week intravaginally.

Fortunately there were long term trials > 10 year clinical trials. These were presented to Dr Torres and he said fine.

Imvexxy was used intravaginally, such a small dose,   And my nephrologist said fine. He has continued to check MRI of my liver + liver studies.  I was blown over with a feather.

I asked my Gyn urologist when I should come off Keflex. He said not for at least 6 months. I have always and still have good gut bacteria.

So now I am patiently waiting. My Gyn-urologist wants to stop my antibiotic in July. I see my nephrologist in Sept, so I will wait.

This is something that can happen with aging. One must have 4 UTI in a year. It comes with aging. I even had a Breakthrough UTI while taking Keflex but since adding Imvexxy no UTI ‘s.  Hurray! These are so painful for me.

I’m not willing to add yogurt, from dairy, to my regime.  Thank you Cath for the info.

By the way Imvexxy is even used for dryness after breast cancer treatments. Coupled with yoga exercises, I am a believer. It makes sense to me. I understand it.

Dear Cath
I will mention this article to my Gyn-urologist. I read it and it’s interesting. I also followed it with some clinical trials that followed. One used probiotics plus Bactrim.

It might be useful when I transition to stopping the antibiotic Keflex 125 mg HS only.

Thank you for the article. In the other discussions do they say the age of the women? Or men?

I’m 73. My urologist is 84.  We are both pretty old.

Dear Cath
The first UTI I had klebsiela aerobacter was the cause. The second was kleibsiela something else. The third time it was E. coli the fourth it was a normal pathogen etc. so for me it was only E. coli once.

hi diane,

one of my friends who has had good results with probiotics is 70 years old. she has used it for vaginitis and uti. only a few years back when she mentioned probiotics to her gynecologist she got blank looks and they would only prescribe all kinds of creams or antibiotics but now she tells me that her gyny introduced an estrogen suppository that had some lactobacillus in it but as she is afraid of estrogen and possible cancer she stayed away from  using it long term. now she says she uses probiotics as maintenance and has good results.

taking probiotics is now considered as one of the health regimes for a lot of issues. for vaginal and urinary tract health the 2 strains most studied are lactobacillus rhamnosus and lactobacillus reuteri.

i dont know if you have read dr william li's book, he is a world renowned physician who served on the medical school  faculties of harvard, tufts and dartmouth. his book has a whole chapter on the importance of good bacteria. he believes that we can use food to beat disease. much like the philosophy of this group. i am straying...

i dont know when your next gyny appt is but you can just research about using probiotics just for your health. yu can try and see if you find any difference so long as your GP or your mayo clinic contacts don't object to it.

hope this helps

hi diane,

i just reread your post and noticed that you have been prescribed antibiotics till july. this will kill all your good bacteria. that is why it is a vicious cycle. nowadays many doctors know that they should prescribe probiotics to their patients after a course of antibiotics.

as you may have read in some of the medical articles i sent you, antibiotic treatment is getting more difficult as our system gets resistant to them. you should be taking probiotics with this course of antibiotics, please email your mayo contacts and hopefully you will get someone that knows about the importance of probiotics and gut health for UTI

Hi Cath,

I was suffering with lots of UTI a few years ago. My Urologist
recommended taking 2 cranberry pills with Vitamin C a day. It has
helped me. They are over the counter so it is something you might
try. I haven't had a UTI since I started doing this.

Good luck, I know how awful UTI can be.

Dear Sharon,
Vitamin C causes crystals to form in urine and cranberry is acidic, Unfortunately this is NOT a treatment for me with PKD, In PKD the crystals form cysts according to Thomas Weimbs.

My gyn-urologist recommended these, I explained what I have explained to you,

Hullo

I have a partner who suffers UTI now and then. Much less so however when we are drinking Kefir in our diet, the water one.
We have included it in our diet for about five years now. I make it weekly, which is simple one in the rhythm. I make one and a half litre batch each time.
It is no use taking pro-bionics when on anti-biotic and as they tend to conflict.
Do one then the other. Anti-biotic’s tend to be acid with low pH.
Kefir can be made with high pH water, and likes minerals.

Just to mention it.

All the best Michael

Take advise before using Bactrim. It is very powerful and can damage kidneys, so needs supervision of a nephrologist. Usually it is only prescribed when the target is known or is otherwise inaccessible via the digestive system. Like inside a cyst say!
It is always advised to identify the bacteria and target a drug than take a shotgun drug.
The most general being Amoxycillin. Yet resistance is built by regular use!

dear diane,

probiotics have to be taken a few hours away from your antibiotics. if one is on a short course of antibiotics like 5 to 7 days, the doctor would recommend that one boosts ones good bacteria at the end of the course with probiotics. however, if one is on a long course of antibiotics, it is very important that one replenishes ones good bacteria. they are ones friends to ward off all the bad bacteria that can take hold in the absence of the good bacteria. so taking a good probiotic with at least 50 billion cfus with mutiple strains would help ones fight against uti. i have read that certain people should not take probiotics eg immunosuppressed persons. it is best diane, that you check with your doctors if there is anything you need to watch out for.

many foods of course have some good bacteria eg sauerkraut, kimchi, yoghurt etc but some may want to stay away from dairy or too much salt.

please let me know what your doctors say. i hope you can find one who is knowledgeable about this subject.

best

She has Ovarian cysts. Which sometimes get infection. Nothing medically can be done unless surgery.
Management works okay mostly. Yet as she gets old the frequency is said to increase UTI anyway. Not sure why?
I think the body does not handle sugars so well as one gets older and they are acidic. That and yogurt. Very french to have yogurt with their sugar first thing as Breakfast. Yet this is my tin pot stab at an answer. The is probably a real one!?

The body drys out as we age including the skin of the nether regions. Hormones make it moist.

Imvexxy is a suppository that odd a very teeny tiny amount. My sister- in-law with breast cancer has been prescribed Imvexxy.

I simply use fermented foods daily. Instead of juicing cabbage as i used to, i now ferment it and use the ferment water to make my own nut and seed yoghurts, hummus etc. I used to regularly get UTIs which on a couple of occasions hospitalized me as it progressed to a kidney infection. I havent had this problem for ages now. I think with PKD we are susceptible to UTIs because we have poor microflora anyway, after all, the kidney cysts are full of mycotoxins. I personally think regular probiotics and fermented foods (because of the high probiotic count) are vital if the body is riddled with cysts full of mycotoxins..

Frank, I use raw apple cider vinegar whenever I feel unwell. It can beat a cold in a day. It is alkalizing to me. It decreases stomache bloat and knocks out kidney pain if I ever get it. It chases away brain fog. It kills shingles.   I know this is a different view than others have but I love what it does for us. — Sheryl Johnson

Apple cider vinegar or any vinegar is not good for PLD liver cysts. It will eventually make them grow and multiple. Try lemon juice instead.

Apple cider vinegar does this to liver cysts as it acts the same as any sugar, yeast, or vinegar.
The after product of this product is vinegar,

It is the by product or the vinegar which makes the liver cysts grow and multiply eventually,

Dear All
I am old as sin! Really old. I never got UTI until I turned 72. My gyn-urologist says this is very common among older oldies.

It is Chronic UTI in the elderly. Some things I could try are probiotics, hormones. and a drug that turns ones urine acidic.  I am trying very hard to keep myself alkaline.

So now I am taking Imvexxy 4 micrograms.  It has been studied for 25 years and it is useful for women with breast cancer (my sister in law) and for those with PLD Polycystic Liver Disease and low and behold Dr. Torres told me it was fine to take it

I was so surprised, I am still take 125 mg of keflex each night plus imvexxy twice a week,

~Diane
