I posted this on Saturday, but I had my controllers wired incorrectly and had to fix them. Now that is done and tested, I completed my spreadsheet of switch results using /opt/pidp1/test.sh

The PDF is a printout of the results for all switches including controllers.

I number switches that have no front panel numbers according to the numbering scheme used on the sense switches - that is starting from the LEFT with 1 (or 0 for the first Extension switch). I did get one "odd" result with the single address switches 11 & 12 as labelled on my diagram when only that switch is flipped up, but it goes away if you flip any other switch as well. It does not seem to affect any operation that I have found to date.

I saw your PiDP-1 annotated serial connector photo which you later said was incorrect.  I would be very interested in a photo with the corrected labeling that you came up with.

Thanks for your consideration.

Here is the serial connector (DB-9 male) rear view with the corrected labels. I re-wired mine up according to this diagram this morning and tested it and it is correct.
