Form Ranger stopped working

I have a simple form which access a Google Sheet containing a list of names. The form has used Form Ranger successfully for a couple of years but yesterday stopped working. I have deleted the range referenced and reset the column containing the names, but although the preview panel seems to reference the correct data the form does not contain the required dropdown list of names.

I have tried copying the data to a new column, bit that didn't work either.

Make sure you don't have any duplicates in your table. Two John Smith records will prevent the table from loading.  Use unique function on your list.

Are you signed onto more than one account?  Try incognito window in Chrome.
