Form Mule Triggers not working

I have a simple form that I am wanting Form Mule to send on submit. I have tried to send on submit and tried the timer trigger.  Neither are working.

Am I correct that it is a Google form?

Try turning off the triggers and closing the spreadsheet.  Then open and reset the triggers.

If that doesn't work, create a new response output spreadsheet and start over.

I have done all of these things to no avail. Yes it is a Google sheet.
