You are publishing personal information such as names and emailaddresses from form replies this way.
If you are to use this form and sheet in some form of production I recommend to start from scratch and use this as an example / testbed.
Second:
Not meeting a condition means the cell in which the contents is going to be used for the merger does not comply.
You merged the <<COLUMN WITH THE EMAILADDRESS>> with a comma ',' and another emailaddess, probably your personal one @
live.com In your sheet I've added a comment to not do this and use the BCC: field for this.
If you need to inform the addressee you received a copy of this mail, just mention that in the body and explain why.
If however you need to address two emailaddresses then if ',' does not suffice you should use the appropriate value separator, which is ';' in my country.
But again, you should 'fix' this if the result is actually undesirable.
If you - or Fatema - is not the addressee use CC: or better BCC: if it holds no relevance to the addressee to know this.

You sign of with TIA, which in international context stands for Transient Ischaemic Attack
which I sincerely hope you do not suffer from and as it caused passing away of my father in law last december made me frown what you mean by it?
