Form Mule Form Edit URL Link not automatically generating

Hello,

I have created a Google Form for my school to help with tracking student conduct data for the school year. I am using Form Mule to send an email to the administrator when the teacher fills out the Form. So far, we have had no issues with the email sending and the tag information being correct. 

To respond to the discipline log, the form edit url will appear in the email and the administrator will click that to fill out their response. The problem I am having is that the form edit url link seems to turn itself off at different times. The emails are sending, but the link is not automatically sending as it should. I will go into Form Mule, check the box on the first screen, then the URLs generate. However, I don't want to do this every time. Any suggestions or thoughts as to why this is happening?

I have attached a screenshot of what the email should look like with the link, and what I am currently experiencing.

See post.  I don't know why the edit option is dropping but I don't recommend it anyway.  Using a separate form for the reporting and commenting means that no one can change the data.
