Simple voting application with node.js

Hi All,

I am looking to create a simple voting application which can collects votes from a bunch of people using a bit.ly url presented to them. Only I will be able to see the results. Just looking to see if there is an example with node.js that's available and can be reused? Would it also be possible to get the ip addresses? Please let me know.

Haven't used mentimeter. Just looking for a github code example but open to ideas.
