File streams

It's been hinted on the other thread that streams are "broken".

Where are streams actually "broken"?
I ran into an issue where it may seem that the stream closing process is possibly not well facilitating file streams. (https://github.com/flatiron/winston/issues/423 ==> https://github.com/joyent/node/blob/e1fec22f9783ad28c35639b3766170c620f2d0be/lib/_stream_writable.js). 

Anyone care to comment?
