Dear experts & ASME Authorized Inspectors,


Our company have ASME certificate of authorization for ASME U stamp
with scop covering
manufacturing activities at shop & field sites controlled by our
company.
We have constructed a pressure vessel at field site owened by our
customer.
Now we have to prepare MDR in U1 form.
My question is againest name & address of manufacturer whether we have
to put field site address or address of our company as apeared on
certificate of authorization.

Please share your experience & views on above.

Name and address shall as per ASME certificate

Common Points to Include in U-1A Notes:

1. Radiographic Examination (RT)

Percentage of the vessel subjected to RT (e.g., “Shell and heads 100% RT per UW-11(a)”).

Acceptance criteria (e.g., “RT per ASME Section VIII, Div. 1, UW-51”).


2. Post Weld Heat Treatment (PWHT)

If applied, mention the temperature and holding time (e.g., “PWHT at 1150°F for 1 hour per UCS-56”).

If exempted, state the exemption clause (e.g., “PWHT not required per UCS-56(d)”).

3. Impact Testing

If required, reference the applicable impact test exemption rules (e.g., “Impact tested per UG-84” or “Exempted per UCS-66”).

4. Material Certification
