Dish end crack during spinning

Dear experts 

Kindly find attached photo of dish end

Seam start & Seam end location I found crack during spinning which is approx 30 mm deep 

Material Sa516 gr 70 

Thickness 50 mm

Weld process SMAW + SAW using E7018-1 & EM12K wire

The photograph shows clear slag entrapped inside and on the sides. Polish the specimen to get a clear picture. Check out your voltage, and travel speed.  Those are the areas of improvement.
