Dish End Spin Hole Block

Dear All, 
Greetings.

WE have manufactured 10% torispherical dish end in house & block dish ends spin hole as per attached photographs. 
Please kindly advise me, is it acceptable as per good engineering practice or not?
Material IS IS2062. Thickness is 10mm. Job - horizontal storage tank

What was joint design?
Is that joint is made full penetration ?
What NDT was followed ?

Hole is accepted as per ASME sec viii. But you have to make "v" On hole and than patch. Also patch should be radiography examination..
