cracke- Moc ss cap thk-2.5 mm

dear expert,

pl. review the attached photo first. let me know, what are the causes for crack developed in weld joints between cap to condenser flange ?
I observed crack at fillet weld joints (partition plate to cap inside surface) - how it was occurred ? would both cracks be intersected from inside ?

your valuable reply will be appreciable.

May be weld sequence is wrong please go to IACS No: 47

what is the service and operating parameters(pressure and temp) . And are u sure that it had occurred in service and is not a manufacturing defect ?
