I haven't used this one on lycra but I have had very good luck with the Woodruff hole punch and grommets. I've used it on cloth, cardboard, and PCB boards (as a makeshift via) and I always get consistent grommets. I like that it is a one piece grommet which minimizes the potential mess-ups from trying to align the pieces.

This summer I've been playing with tensile fabric structures stretched within tetrahedral frameworks. http://www.thingiverse.com/Fredbartels/designs
I've been tying knots directly around the fabric but wonder if using eyelets (little grommets) might be easier for kids. Anyone have experience using them with spandex?Thanks,Fred
