When updating a bunch of plugins, if the same plugin is updated twice, the second time it appears as immediate active (see attachment).

I'm pretty sure this is a regression, but it's been there quite a while (1-2 years?).

FYI Jesse Glick I think you've worked in this area; might even have been a change of yours that caused this.
