Basically have a need to have it when a job is disabled there's a reason associated with why.   Also trigger and external system when it is.

Is there such a plugin and/or where do I hook in to create this extension?

This would be a using question, I recommend asking again on community.jenkins.io or users mailing list

I believe disabled is a property on the job itself, as for where/how to handle a reason, you'd probably want to make a new action with a new http post handler, which disables the job and job.addAction(yournewclass)
then render that action however/wherever you want.
