CSharpGrpcGenerator: document additiona flags

Hi,

by looking at the csharp_plugin.cc source code I discovered some additional parameters that can be specified via commandline invocation as follows:

--grpc_out=internal_access,no_client,no_server:C:\my\out

Are these parameter documented somewhere?
