Any idea where it is coming from? Have you installed an Add-on, or started running a script off a trigger?

That looks like a Labnol script. I believe Amit had some examples (https://www.labnol.org/search/?q=unsubscribe) and it may also be a part of his Email Studio or other great add-ons. Just a hunch.
