Tank only fills up 1/4.

I can't for the life of me understand why the boiler will only fill up to just below the heating element. Only 1/4 of a tank. Even when pushing the manual fill button.

Things I've done so far:

Disassembled the tank/lines
Disassembled softener and lines
De-scaled everything
Removed the softener cartridge (shot)
Inspected the water intake valve

The only thin I haven't checked is the pump which comes on when you press the brew button.

Any suggestions are greatly appreciated.

Just a thought:  You didn't mention replacing the vacuum breaker (vent valve) on top the tank.   When you fill the tank are you able hear the air venting off?

Yes, the steam pressure release valve works. I removed the safety valve (the tall one in the middle very top) with the water on to fill the tank manually. Still wouldn't fill itself.

Is it possible the machine has scale in it, clogging the water supply?

I de-scaled all the lines. No blockage whatsoever.

The probe inside the water level tub controls the autofill, anywhere over MIN submerges the element.  No manual fill is troubling, could water be leaking into the heat exchanger if it is damaged close to the bottom?  I have only seen the other way around - HX leaking through boiler.

If there is a hole or break in the heat exchanger running through the boiler, the tank would fill completely as soon as you applied line pressure, so I don’t think you have to worry about this possibility.  

I suspect the manual fill part of the 3 way valve is not working, as you should be able to fill by this method. That valve can be quite hard to push in and may be sticky once you get it to move, but when engaged you should be able to hear it filling up. I just tried mine after not having used it in years, and the initial force to get it free was quite high.  Fortunately, it didn’t stick open for me. 

Some other things to look at:

Site gage fill sensor
Solenoid that opens the fill valve (in the three way valve)
Overpressure valve (within the 3 way valve).  If this is stuck open or caused to open during the fill cycle, the fill water dumps to the drain cup.

Maybe you could walk us through in detail what happens when you turn the machine on.
1) Can you see the level sensor in the level gauge? What level is it set to?
2) When you turn on the machine and the water level is below the sensor, does the pump come on after a few seconds?
3) Can you disconnect the pipe that fills the boiler? Does water come out of that pipe when you push the manual fill button?

Alright!! The tank now fills when you press the auto fill button!! I disassembled the water intake valve, de-scaled and cleaned all pieces and reassembled it. It works perfectly. However it does not auto fill. I suspect the probe in the water level gauge is damaged. I disassembled the gauge and cleaned the housing and the probe, reassembled. We'll see it that works.
